Registration and Fees

TEE Preceptorship Registration:

  1. Register online: Please choose the month you wish to register from the calendar below to be directed to the CME registration website.
  2. Look for what Preceptorship three day sessions are available
  3. Book your preferred Preceptorship session
  4. Sign-up for consideration on a waiting list for sessions already booked
  5. Special Duke rates at nearby hotels
  6. Printable itinerary for the session sent to you by email
  7. Credit card and other convenient payment options available. See payment options below.

Tuition:

Registraion for the intraoperative TEE preceptorship is USD $4,500 for the three day session.

Payment Options: MasterCard & Visa Accepted

Online: Accepted methods of payment are Mastercard & Visa credit cards ONLY. Registration confirmations will be provided online immediately following registration. If you have problems with the online registration and payment website, please contact the Duke CME Office at cme@mc.duke.edu or call 919.401.1200.

Offline: Accepted methods of payment are Mastercard, Visa and Personal Check.  If you would prefer not to register online, you may download the form and mail it with your payment. Forms with credit card payments may be faxed to 919.681.8994 attn: Jaime C. Cooke. Mail checks to Duke University Medical Center, DUMC, Anesthesiology (TEEPS), Box 3094, Mail #49, Durham, NC 27710. Registration confirmations will be mailed to the address provided. After receipt of the check, dates requested will then be blocked out, if still available.
 
Payments must be made at least 6 weeks prior to start date. If paying by check, please ensure that the payment along with the paper registration form is submitted at least 6 weeks prior to the start date for the preceptorship (postmark on payment packet will be used). If payment is not received at least 6 weeks prior to the start date, the registration will not be processed and the dates may be provided to the next individual on the waiting list.
 
Additional days:

If preceptees wish to extend their preceptorship experience beyond the three day session, they may do so at a charge of USD $1,250 per day for a maximum of two subsequent and consecutive days. Preceptees must indicate at the time of registration and include the additional amount with the registration fee. The extended days are not a part of the CME-approved course and CME credits will not be awarded for activities on extended days.

Cancellation/Refund Policy:

A written notice of cancellation must be received 6 weeks prior to the start of this activity to receive a full refund. A $215 cancellation fee will be assessed after that date. No refunds will be given if cancellation requests are received within 3 weeks prior to the start date of this activity. If original registration fee paid by credit card a 3% cancellation fee will be assessed on ALL cancellations.
